Credentialing Details for JHHS:

  1. Application – Needs to reflect last 7 years work history and education history – access via the online application instructions below
  2. CPR (Please be sure to send both the front and the signed back of the card)
  3. ACLS / Advanced Certifications (Please be sure to send both the front and the signed back of the card)- If Applicable for specific unit.
  4. Current MD License or Compact License
  5. IDs– 2 Forms required (drivers license, passport, SS card, birth certificate, etc)
  6. Professional ReferencesJHHS requires a current reference from an Asst. NM or above from your two most recent positions. They will make no exceptions to this policy.

  7. Annual Physical– Please refer to our Statement of Good Health form.
  8. Annual PPD
  9. MMRVs- Either positive titer records or documentation of 2 vaccinations.
  10. Seasonal Flu Vaccination- JHHS does not accept a declination.
  11. HepB- Positive Titer record or signed Declination.
  12. Tdap- Must be from within the last 10 years. JHHS does not accept a declination.
  13. Annual Comprehensive Core Competency Test and NPSGs
  14. Annual Competency Test
  15. Annual Medication Test
  16. Annual Skills Checklist
  17. 30 Day Drug & Alcohol Screen- This would be set up to be completed via Labcorp.
  18. Other:
    1. Annual Fit Test (Please refer our Fit Mask Form)- You will just need to fill out the top portion and check off one of the boxes at the bottom, send back to us, and then our clinical director will review and sign off at the bottom.
    2. JHH Forms- Would be assigned via online application for completion
    3. JHHS MyLearning Modules- Once pre-approved, the facility would set you up with a log-in to complete these modules. They include education re: HIPAA, Fire Safety, Bloodborne Pathogens, Discrimination and Harassment, and other Required education topics.
    4. Trauma Education Requirement-
      1. PREFERRED- Prior to start of assignment, shows proof of completion of a MD Trauma Center 16 hour Orientation Course or one of the 16-hour basic courses listed below:
        • Adult Care Providers: TNCC, ATCN,TCAR, CATN,
        • PEDS Care Providers: PCAR, ENPC, ATCN,TNCC
      2. ACCEPTABLE- Prior to 27th week of assignment, attends one of the 16-hour basic courses listed below:
        • Adult Care Providers: TNCC, ATCN,TCAR, CATN,
        • PEDS Care Providers: PCAR, ENPC, ATCN,TNCC
      3. Trauma Continuing Education
        • Once initial course has been completed:
          1. MS-Obtains 4 trauma CE hours each year for non-critical care RNs caring for trauma patients.
          2. ED, ICU, PACU, IMC, OR- Obtains 8 trauma CE hours each year for critical care RNs caring for trauma patients.
